PROGESTIN CHALLENGE TEST RESPONSE ON SONOGRAPHIC ENDOMETRIAL THICKNESS

Introduction
After menopause the threhold value of the endometrial echo associated with atrophy of the uterine mucosa range 4-5 mm
The progestogen challenge test has also been recommended by some investigators.

Objective
To determine the response to the progestin challenge test on endometrial thickness < 5mm
sonographically.

Methods
Progestin challenge test were performed on 128 .patients with secondary amenorrhea with micronized progesterone 200 mg or noretisterone acetate 10 mg x 10 days. Ultrasound endometrial thickness and serum estradiol levels, FSH UI/ml,TSH and Prolactine were determined.

Results
Positive challenge test with withdrawal bleeding was found in 15 % of patients with endometrium < 3mmm., and 58 % of patients with FSH > 40 UI/ml and Estradiol < 20 pg
There was a positive correlation with estradiol on ultrasound endometrium thickness.

Conclusion

Measured endometrial thickness appears to be a better assay of endometrial estrogen response than estradiol levels as good as progestin withdrawal bleeding
The progestin challenge test in amenorrheic women is a good method for judging the severity of ovarian dysfuntion
